Ours is a team-based model of
care - led by a personal
physician who provides
continuous and coordinated care
throughout a patient's lifetime
to ensure positive health
outcomes. This Patient-Centred
Medical Home style
practice is responsible for
providing all of a patient's
health care needs or
appropriately arranging care
with other qualified
professionals.
We aim to address all factors
that contribute to health,
healing, and disease treatment,
blending advanced clinical
practice with proven
complementary therapies.
We coordinate clinical care,
individualized health coaching,
and therapeutic support to
address the physical, mental,
and emotional needs of all our
patients, through the combined
effort of our physicians,
